Department Description:

CHA PACE is a Program of All-Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE's high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services. 

Job Summary:

Under the direction of the Operations Supervisor and Home Care Manager, and as a member of the PACE Interdisciplinary Team (IDT), the Home Health Aide provides personal reliable transportation in private vehicle to homes of PACE participants, as well as assistance with Activities of Daily Living in PACE participants’ private homes, apartments, etc. as prescribed in their plan of care and daily assignment. 

Responsibilities:

Works independently in the private home, apartment, etc. of PACE participantsProvides own reliable transportation to the private home of PACE participantsAssists participants in bathing, including complete, partial, or sponge bathing in tub or showerAssists or provides laundry service as assignedEnsures participants' home/environments are clean and tidy in order to promote health and safetyCommunicates effectively with management, peers, and healthcare team membersCompletes daily home care or day center assignment, providing support and assistance with activities of daily living (ADLs)

Qualifications:

High school diploma or equivalentAt least one year working with an older adult population requiredAt least one year experience working as a personal care attendant, home health aide, or other direct care professional requiredMassachusetts drivers license and personal transportation requiredHome Health Aide Certification in Massachusetts highly preferred
