Position Summary

The Home Health Director of Quality Outcomes is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Director of Quality Outcomes assesses the quality of care provided to home health patients, families, and staff. S/he audits documentation, compiles and reviews clinical performance data for quality and regulatory compliance, participates in activities to support survey readiness, provides education to support clinical development and positive outcomes, and works closely with Program leadership.


Position Specific Responsibilities

Owns monthly Home Health Clinical Scorecard, documented outcomes data, POC effectiveness and implementation, ensures all reviews are completed and accurate.Analyzes information gathered from audits and outcome data and shares findings with home health leaders along with recommended solutions for identified needs.Performs focused reviews/audits at regular intervals to assess for compliance, support QAPI initiatives, and ensure constant survey readiness, or as requested by the fiscal intermediary.Assesses and/or reviews clinical documentation to ensure documentation reflects the quality of care and services provided in addition to Medicare/Medicaid regulations.Ensures QAPI is accurate and complete.Compiles quality data on quarterly basis and submits summary to Regional Executive of Clinical Operations for Governing Body reporting.Participates in the development and update of patient care policies, procedures, and emergency procedures.Provides education to team members and clinical teams to support improved OASIS documentation and optimal outcomes.Assists with Plan of Correction audits following internal/external surveys/audits.Reviews clinical documentation of OASIS assessment and plans of care and identifies educational opportunities to improve understanding of OASIS and regulatory guidelines.Reports deficiencies to appropriate staff and supervisor for corrections.Collaborates with Home Health Regional Executive of Clinical Operations in deficient areas to monitor the effectiveness of resolution.Collaborates effectively with Home Health Regional Executive of Clinical Operations, Director(s) of Clinical Services and members of senior leadership to assure clinical excellence in each location.Reports deficiencies to appropriate staff for corrections and follows up to ensure completion.Assists program leadership with development and implementation of plan of correction following internal/external survey/audits, updates supervisor when a plan of correction validation intentional site visit is to be completed.Coordinates with Regional Executive of Clinical Operations during survey and collaborates with program leadership to guide, direct and support survey activities and plan of correction.Supports program leaders to ensure constant state of survey readiness.Participates in performance improvement programs and assures oversight of performance improvement plans.Communicates proactively when schedule changes are needed.Models and facilitates communication between team members including medical records, business office, intake, clinical operations and leadership.Provides input into program service recovery efforts.Supports clinical onboarding, assures annual competency and provides training for team members to drive clinical excellence.Solicits and incorporates internal team member feedback into quality efforts and performance improvement.Travels intermittently, as needed, to support program locationsPerforms other duties as assigned.


Education and/or Experience

Associate of Science in Nursing or completion of an accredited Physical or Occupational Therapy education program required.Advanced degree in Nursing, Physical Therapy, or Occupational Therapy preferred.Minimum of three (3) years of recent home care experience required.Minimum of one (1) year experience in home health QAPI or QA related field preferred.At least one (1) year of experience with point of service electronic documentation, HCHB preferred.


Skills

Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.


Certifications, Licenses, and Registrations

Active and unencumbered Registered Nurse, Physical Therapist, or Occupational Therapist license in state(s) of employment required.Current certification as a home care-coding specialist and OASIS certification preferred.


Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 25 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus.
