We’re looking for a dedicated Home Health Intake Coordinator to join our team! In this essential role, you’ll manage agency referrals, schedule both new and ongoing visits, assist with medical record requests, and handle service authorizations and order management. Prior experience in Home Health is a must to help ensure a smooth intake process and seamless coordination of care. As part of Fellowship Square, Christian Care, we share a deep commitment to providing compassionate care and ongoing support for our residents.

Responsibilities Assist in coordinating Home Health Services for patients, in a timely manner.Process incoming referrals via fax and email.Verify benefits via Inovalon software, insurance portals, AHCCCS website, and telephone confirmation. Obtain initial authorization when applicable as well as ongoing authorization.Confirm following physician for Home Health.Staff appropriate clinicians per physician orders.Upload referrals and enter patient information into EMR. Initiate welcome calls with patients/family verifying accurate referral information.Initiate Tiger Connect Chat with clinicians.Manage ongoing Tiger Connect requests from clinicians.Manage clinical email threads.Manage medical record requests and orders for signature.Maintain a good working rapport with interdepartmental personnel, as well as other departments. Qualifications A high school diploma or equivalent required.Two (2) years of experience in a health services organization required. Knowledge of medical terminology, aging services, and Home Health required. Computer proficiency in Microsoft Office products required.Must have strong organizational and multi-tasking skills.Must be able to communicate effectively verbally and in writing.Must possess excellent customer service.
