Location: Regional Preceptor for Winter Park, Casselberry, Altamonte Springs, Maitland, Kissimmee territories.
The role you’ll contribute:
The Regional Preceptor reports to the Program Director of Connected Care Coordination. Focuses on inspiring hope and improving the quality of life for those entrusted to our care by managing the day-to-day operations and care integration of our Connected Care Coordinators within our network. The Regional Preceptor works in collaboration with Director to educate active care coordination team members, onboard and orient new hires. This individual will be responsible for training, counseling, mentoring and evaluating the Connected Care Coordination team.
The value you’ll bring to the team:
Develops and coordinates the orientation process for assigned areas, meets with orientees, preceptors and members of leadership team as needed.
Develops training schedule with alongside director/HR/supervisor
Updates training material based on evidenced-based practices, industry changes, regulations, and guidelines.
Introducing new hires into their new roles and responsibilities, in-field training and evaluations on processes and documentation
Offers learning opportunities that promote critical thinking, clinical reasoning and problem-solving in job role.
Actively participates in quality and safety initiatives to ensure team members are improving practice so quality and safety standards are met.
Collaborates with Corporate Education to assess ongoing competence of clinical team members.
Plans, develops, coordinates, and evaluates job specific programs to maintain or increase the skill and knowledge of team members.
Ensures adult learning principles are integrated into all programs
Facilitates and supports team member involvement in professional and organizational activities.
The expertise and experience you’ll need to succeed :
Minimum qualifications:
Associate's of Nursing
5 years Related Home Health Experience
Active American Heart Association BLS
Registered Nurse – State Licensure and/or Compact State Licensure In Florida
Valid in state Driver’s License with current car insurance
Preferred qualifications:
RN Home health experience needed
Competent in basic and advanced nursing clinical skills
Excellent teacher and educator on medically complex patients and developing home health care plans
Willing to preform Home Health nursing visits to meet all patient care needs
