Position Summary:

The Supervisor is responsible for supervising, planning, and coordinating the delivery of individualized care and comprehensive service to patients and families. The Supervisor is also responsible for coordinating training, scheduling, and supervision of nurses and aides providing services to patients in the home setting. The Supervisor supports the following major Community Care processes: admission, discharge, referral, delivery of care, complaints, staff orientation, training and development, and billing

Work Schedule: M-F 8a-4:30p

Required Qualifications:

Education, Experience or Equivalent Combination: Associates degree Licensure/Registration/Certification: RN License in MN  Individuals are responsible for the travel arrangements required to meet the expectations of the position Knowledge, Skills, and Abilities: Must possess knowledge of reimbursement regulations  Demonstrate knowledge of age-related growth and development principles necessary to provide appropriate services and assure safety of patients Proficiency using Personal computer

Preferred Qualifications:

Minimum 3 years’ experience in home care  BSN/PHN  Previous supervisory experience preferred Excellent written and oral communication skills and ability to speak to groups Committed to the philosophy of customer service Ability to problem solve and interface with primary customers Ability to relate to health professionals and to work with and through others
