Homecare Physical Therapist - Per Diem and Full Time (Suffolk County - Field Based) Suffolk County, NY, USA Req #11781 Tuesday, November 5, 2024 We are expanding our foot print and seeking compassionate Physical Therapist to provide home-based physical therapy services. Our Therapist contribute to improving mobility and enhancing the quality of life to our members. Physical Therapist Responsibilities: + Travel to and from patients' homes, providing physical therapy as ordered by a physician in accordance with the designated plan of care. + Design treatment plans that achieve results through personalized, 1-on-1 individualized care. + Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit. + Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons + Diagnose movement dysfunction by listening to patients and observing them as they move about + Develop quality, personalized, evidence-based care intervention programs that improve outcomes + Use hands-on therapy such as massage to ease patients’ pain and provide stimulation to promote healing + Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly + Instructs family members, Home Health Aides, and other caregivers in appropriate physical therapy techniques and what to expect from the recovery process Qualifications: + Current Physical Therapy Licensure in NY State + Graduate of an accredited PT program + Proven work experience as a physical therapist + Intensive education and clinical expertise + Ability to manage patients with different types of personalities + Current knowledge of treatment practices + Excellent interpersonal communication skills Salary: + $80.00 per visit Other details + Travel Required Yes + Travel % 100 Apply Now + Suffolk County, NY, USA
