Do you want to be part of a team with a new innovative practice that helps serve the Wilmington, DE community?
ChristianaCare is looking for an energetic and flexible NP or PA that has a passion to care for highly complex patients with our Mobile Health Services and Medical Respite Program at the New Castle County Hope Center.
 
Our Mobile Health Services van provides comprehensive family and primary care to individuals throughout the Wilmington Community.  This includes wellness visits, sports physicals, chronic disease management, wound care, vaccines, sick visits, and more!  The Mobile Van has a rotating schedule through various community centers where patients can access care via walk-ins or scheduling an appointment.
 
The Medical Respite Program at the Hope Center is designed for the individual who struggles with a medical or behavioral health condition and in addition, may have suboptimal physical and social health.  In a welcoming environment, the program offers multidisciplinary treatment, including psychiatric and behavioral therapy care, chronic condition and general health treatment, and comprehensive social health evaluation and intervention.

EDUCATION AND EXPERIENCE REQUIREMENTS:

Master's degree in Nursing or Physician Assistant Studies

License or license eligible in the state of Delaware 

Board certified as a Nurse Practitioner or as a Physician Assistant

Minimum of three years of experience working with individuals with complex behavioral health conditions preferred

Prescriptive Authority, DEA registration, BLS Required
