Hospice Chaplain PRN (Horry County)

Founded in 1906, McLeod Health is a locally owned and managed, not for profit organization supported by the strength of more than 900 members on its medical staff and more than 2,900 licensed nurses. McLeod Health is also composed of approximately 15,000 team members and more than 90 physician practices throughout its 18-county service area. With seven hospitals, McLeod Health operates three Health and Fitness Centers, a Sports Medicine and Outpatient Rehabilitation Center, Hospice and Home Health Services. The system currently has 988 licensed beds, including Hospice and Behavioral Health. The hospitals within McLeod Health include: McLeod Regional Medical Center, McLeod Health Dillon, McLeod Health Loris, McLeod Health Seacoast, McLeod Health Cheraw, McLeod Health Clarendon and McLeod Behavioral Health.  


1.    Provides spiritual and emotional support to the Hospice patient and family.
2.    Assesses patients/families accepted on the Hospice Program to determine spiritual and emotional needs within 5 days of admission 80 percent of observation.
3.    Attends interdisciplinary team (IDT) meetings.
*Recognizes signs and symptoms of the actively dying patient and communicates this information to team members.
4.    With the patient/family’s permission, informs patient’s minister, priest, or rabbi that patient is receiving      Hospice care. Coordinates community spiritual care.
5.     Honors and encourages the patient/family relationship with their own minister.  Coordinates minister documentation in patient’s chart.
6.    Provides continuing support for families through coordination of bereavement care services for 12
months after patient’s death. 
7.    Provides assistance as needed or requested at time of patient’s death (i.e. funerals, worship services.

 


Qualifications /Training:
•    Ordained clergy (if no related degree)
•    Preferred Education:  Bachelors or Masters Theology.
•    At least 2 years of pastoral experience preferred. Participation in educational opportunities dealing with death and dying. 

Licenses/Certifications/Registrations/Education:
Must have a valid South Carolina drivers license and a vehicle available at all times.
