SUMMARY OF JOB The Hospice Clinical Manager and Education Specialist is responsible for the daily operations and supervision of the interdisciplinary clinical team to include, but not limited to, nurses, therapists, schedulers. Delivers quality patient care by coordinating, implementing and evaluating Hospice services evidenced by positive outcomes. Assumes 24-hour accountability for patient care functions and the direction of the patient care team, and the evaluation and treatment of hospice patients when needed. In addition, this position oversees all aspects of the Hospice team’s professional development to ensure the ongoing excellence of nursing practice. Proactively identifies knowledge and/or skill gaps and implements remediation to help staff grow and refine clinical expertise, reviews and updates policies, and ensures best practice and quality of care. Supports quality improvement initiatives, regulatory and accreditation projects and processes as needed. Perform on-site supervisory visits with team members at least annually. A primary way staff connect to our mission is by collaborating with all team members in promoting an environment that provides quality, compassionate, holistic, patient-centered care and supports all internal and external customers RECRUITMENT REQUIREMENTS BSN; Master’s Degree in Business or Education Minimum of 5 years of clinical experience and 5 years of direct hospice management experience Possess working knowledge and understanding of Home Care and Hospice regulations and compliance requirements with local, federal and regulatory agencies Ability to demonstrate professional competencies in a home care setting and to function as a member of an integrated healthcare team Proficiency in utilizing technology (cell phone, computer) Ability to travel within service area and throughout RHS entities. Demonstrated understanding of clinical practice, a passion for teaching and a commitment to continuous improvement in healthcare. Proven understanding of adult learning theories and principles LICENSE AND REGULATORY REQUIREMENTS: Current PA/NJ Professional License, Valid Driver’s License, CPR Certified
