As a Hospice CNA, you will:Patient Preparation & Communication: Review care plans, check in with patients, introduce self, explain tasks, and maintain respectful communication with patients, families, and staff.
Direct Patient Care: Assist with personal care, mobility, and comfort, including safe transfers and support with meals and hydration.
Monitoring & Reporting: Observe and promptly report changes in patient condition to the RN, suggesting care plan updates as needed.
Emergency Response: Respond calmly and appropriately to emergencies, ensuring patient safety.
Team Coordination: Communicate effectively with the hospice care team, patients, families, and external contacts to ensure seamless care.
Documentation & Privacy: Maintain accurate records and adhere to HIPAA guidelines to protect patient privacy.
The role requires travel, participation in on-call rotations, and a compassionate, proactive approach to end-of-life care.
What You’ll Need to Succeed:Active CNA license in Michigan
A positive attitude and passion for delivering quality patient care.
Strong communication skills and reliability.
Dedication to going above and beyond for patients.
Reliable transportation to perform your duties.
Highly preferred to live in service area; less drive time for you!
