Hospice Community Liaison
Integrity Home Care + Hospice, in partnership with BAYADA Home Health Care, is looking for a Hospice Liaison, who will develop and strengthen relationships within designated skilled nursing centers and senior living communities. Where appropriate, this individual will interact with referral source staff members, potential patients and families, and hospice team members to educate and promote the organization’s hospice services and coordinate the initiation of hospice care. Territory: Springfield and Joplin, MO MINIMUM QUALIFICATIONS: 1. Exemplifies characteristics of The BAYADA Way: compassion, excellence and reliability. 2. Four (4) year college degree. 3. Minimum of one (1) year experience in hospice, home health, senior living, and/or marketing. 4. Demonstrated record of strong interpersonal skills. 5. Demonstrated record of building and strengthening relationships with key decision makers. 6. Demonstrated record of goal achievement. 7. Demonstrated record of providing excellence in customer service 8. Ability to work well in a team environment and matrix organization. 9. Competence in business office technologies required to perform job functions. i.e., PC/ laptop, tablet, MS Office applications, smartphone. PRIMARY RESPONSIBILITIES: 1. Demonstrate and communicate the core values of BAYADA and The BAYADA Way. 2. Develop working knowledge of BAYADA’s mission, services, people, organization, policies and procedures. 3. Develop and strengthen relationships with new and existing referral sources, physicians, and other healthcare professionals. + a. Plan, execute, and carry out marketing activities as outlined as part of the business planning process. + b. Work in collaboration with Marketing Manager(s), and Program Managers(s) to position BAYADA Hospice as a partner in designated referral source accounts, ensuring comprehensive implementation of the hospice service offering. + c. Identify referral source obstacles and participate in implementing resolutions. 4. Upon receiving referral to hospice, meet with patients and families to review the hospice benefit, answer questions, and coordinate with office personnel for the timely intake and admission to hospice care. + a. Identify obstacles with patient/ family satisfaction and participate in implementing resolutions. 5. Regularly use all business development tools and resources including, but not limited to, marketing collateral, customer facing outcomes reports, and third-party data sets. 6. Consistently document all marketing and patient/ activities at the direction of the Area Sales Director. 7. Attend and participate in all business development meetings at the direction of the Area Sales Director. 8. Demonstrate mastery of hospice eligibility criteria. 9. Demonstrate solid performance or exceed performance standards in key job dimensions/attributes as defined on the Performance Appraisal for Office Staff. 10.
