Inova Health System’s Palliative Medicine & Comprehensive Care program is seeking a dedicated Hospice Coordinator to join our dynamic team! The role will be remote or on-site, part-time weekends, flexible. Ideal schedule would be a combination of 2-10s (8a-6p) Fri, Sat, Sun, & Mon.
The Hospice Coordinator counsels hospitalized patients and their surrogates on the hospice benefit and related services, offers agency choice, communicates referrals, and collaborates with hospital case management to support seamless transitions in care for those who need it most. The ideal candidate has professional experience in hospice and acute care and is a strong and compassionate communicator.
Inova Hospice Coordinators make a difference, and families report high levels of satisfaction with their interactions. Inova Hospice Coordinators are part of the larger Inova palliative interdisciplinary team which spans all five Inova hospitals and includes physicians, advance practice providers, clinical therapists, a nurse, and a chaplain.

Explains hospice philosophy, levels of care, goals and services to patients/families, nursing staff and providers to ensure understanding of principles of care. Communicates referrals to hospice agencies as indicated. Maintains relationships with hospice agency staff to facilitate communication and transitions of care. Demonstrates knowledge of community resources to meet patient needs. Provides in-services to providers, nursing staff and Case Management as appropriate. Assesses knowledge of staff and provides information on an individual basis when indicated. Utilizes standards, guidelines, and pathways for care delivery. Identifies trends and develops/implements processes and plans for addressing these trends appropriately. Assists with compliance aspects related to hospice. Collects appropriate outcome and referral data and reports to leaders on a regular basis. Performs other duties as assigned.Additional Requirements:

Education: Bachelor’s degree in nursing or social work (BSN or MSW) Experience: Two years of public health/community health, medical/surgical, palliative, or hospice nursing/SW experience. Hospital based nursing/SW experience. Certification: If BSN, must be licensed as Registered Nurse in the Commonwealth of Virginia. BLS from the American Heart Association.
