Position Summary / Career Interest:Provides clinical leadership and oversees day to day operations, staffing and staff coverage and overall staff performance to goals. Ensures consistent delivery of high-value, quality hospice care, management of staff, and clinical services for patients receiving hospice care. Identifies and implements best practices and cost savings. Collaborate with others to coordinate services, streamline and standardize workflows and procedures, and/or drive positive outcomes.

Responsibilities and Essential Job FunctionsDevelops and maintains knowledge of hospice care and compliance with the Hospice Conditions of Participation as a primary responsibility. Develop and maintain knowledge of home health as secondary. Provides nursing input towards the initial and updated interdisciplinary plans of care for clients and families. Part of an interdisciplinary team that completes chart audits to assess the adequacy of the plan of care. Advocates for necessary positive changes in interventions and the plan of care to meet identified goals. Supervises the planning, coordination, and delivery of the Interdisciplinary Team interventions. Participates in weekly team meetings providing leadership and nursing expertise to facilitate problem solving and the delivery of team support in a productive timely manner. Maintains contact as indicated with Medical Social Services, Chaplain, Physician, Nursing personnel, Aides, Therapies and Volunteers for collaboration to provide holistic care. Mentors clinical staff to use the full team for optimum patient care and economical use of resources. Provides nursing input and collaborates to determine the need for special care (respite or general inpatient care, or the need for transfer to facility) and facilitates and assists in a smooth transition from one location or level of care to another. Maintains clinical proficiency in symptom management and teaches/mentors’ staff to be responsive to client’s needs, per physician orders and agency policy in a timely, cost-effective manner. Advocates the patient’s right to be as comfortable as possible with consistent management of physical, emotional, and spiritual discomfort by observing and teaching staff and overseeing the patient’s response to the plan of care. Functions as an available resource and ensures competency of nursing staff in symptom management, medication administration, and patient care by developing, teaching, and observing competencies, testing, doing supervisory visits, and chart review. Teaches, mentors, and supervises new Hospice House onboarding and clinical orientation. Assists Intake Coordinator when needed in responding to referrals and provides clinical guidance to eligibility and medication questions. Provides nursing visits to prospective clients to evaluate the condition and appropriateness for services. Provides direct nursing care and support in the field and hospice house, when needed. Serves as a Liaison between the hospice clinical teams, Director, and Executive Leadership as it relates to staffing, policy and procedure and performance improvement. Develops and maintains relationships with contracted facilities, acting as a liaison between facilities and Agency. Coordinates the provision of services to patients in these facilities. Interacts effectively with patients and staff through both oral and written communication and supportive presence. Exhibits effective listening skills. Promotes and contributes to a positive, problem-solving environment. Maintains a professional attitude and observes proper ethics. Reviews patient/family/physician/facility satisfaction surveys and uses feedback to make program improvements. Provides clinical supervision of nursing personnel and participates in annual performance evaluations. Acts as a role model, resource, and mentor, as appropriate and needed. Fosters professionalism and staff development and satisfaction. Must be able to perform the professional, clinical and or technical competencies of the assigned unit or department. Required Education and ExperienceAssociates Degree in nursing from an accredited college or university. 3 or more years of nursing experience.
Preferred Education and ExperienceBachelors Degree in nursing from an accredited college or university.
Required Licensure and CertificationCPR/ AED/ BLS - Other BLS from American Red Cross only. Licensed Registered Nurse (LRN) - Multi-State - State Board of Nursing
