Family Hospice is hiring a Hospice Nurse Case Manager to join its team in Erie!

This is a Full-Time position working mainly Monday-Friday, noon to 8:00 PM. On-call shifts and rotating weekends are required.

Responsibilities:

Provide nursing services in the patient’s place of residence following physician plan of care and agency policies. Ability to assess and interpret patient data needed to identify requirements relative to the population Establishes and maintains collaborative relationships with physicians, other health care providers, patients and families. Documents timely in the electronic health record Participates in on call hours coverage
