Registered Nurse (RN) Case Manager – Hospice CareNew Opportunity Due to Growth – Join Our Expanding Team!
At Elara Caring, we’re growing and seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our team and make a lasting impact on the lives of our patients. As part of our growth, we’re adding a Case Manager to our expanding Hospice service line to help us continue delivering high-quality, patient-centered care across the continuum. The RN Case Manager provides care to hospice patients in their homes, working to ensure that each patient receives the right care, at the right time, and in the right place. With the addition of this new position, we aim to enhance our team and better serve the growing needs of our patient community.
Schedule: Monday through Friday, 8:00a to 5:00p
Territory: Eaton County

What You’ll Do:Lead patient care and coordinate services across the hospice continuum.
Work directly with patients and families to develop care plans that align with their needs and preferences.
Provide hands-on clinical care while also managing a team of healthcare professionals to ensure comprehensive patient support.
Collaborate with physicians, social workers, and other team members to address the physical, emotional, and spiritual needs of patients.
Ensure timely and accurate documentation, meeting regulatory and organizational standards.
Support patient families and caregivers, offering education and resources as needed.
What’s Required:Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate in Nursing).
Current State License as a Registered Nurse (RN).
Minimum of 1 year of experience in a clinical care setting.
Experience in hospice or home health care is preferred.
Strong nursing skills and ability to manage multiple priorities and deadlines.
Reliable transportation to visit patients in their homes.
