Role Overview:As a Medical Social Worker with Elara Caring, you will:
Deliver vital social work services, including counseling and support, to hospice patients and their families.
Help patients and their loved ones maximize coping abilities by offering guidance, grief assessments, and bereavement support.

On an Average day, you will:Aligns Activities with Vision: Ensures all tasks support Elara Caring’s vision and Home Health leadership.
Patient Assessment: Evaluates patients' psychosocial, financial, and environmental needs through documentation and direct observations.
Social Work Evaluations: Conducts initial and follow-up social work assessments for each patient visit.
Communication & Reporting: Shares significant changes in patient status with the Supervisor, physician, and care team; reports visit outcomes and safety issues daily.
Safety Planning: Develops and implements safety plans using available resources for patient and family.
Plan of Care Management: Participates in creating, verifying, updating, and ensuring adherence to the Plan of Care for continuity and quality.
Physician Orders & Documentation: Documents physician orders for additional visits or care changes, ensuring all actions meet standards and physician certification.
Skilled Interventions: Provides social work interventions as per professional standards and the physician’s orders.
Patient and Family Support: Educates and involves the patient and family in following the Plan of Care to address social work needs.
Qualifications:Master’s or Doctoral degree in Social Work from an accredited program (Council on Social Work Education)
Active state Social Worker license
Minimum of one year of experience in a healthcare setting
Strong social work skills aligned with industry standards
Valid driver’s license, insurance, and reliable transportation
Ability to travel up to 50%
