The Mary T Family of Companies is seeking a part-time Social Worker to work within our Hospice line of care.
The Hospice Social Worker coordinates and delivers a service program designed to meet the psycho, social, emotional needs of the patient and family, helping them to adjust to the changes related to their illness and related end-of-life care planning.
ESSENTIAL FUNCTIONS AND BASIC JOB DUTIES
Ensure the patient and family needs are met through proper delivery of care and services. Ensure all psycho, social, emotional needs of the patient are identified and documented. Complete initial psychosocial assessment and social history within five days of the election of hospice care. Participate in the Quality Assessment Performance Improvement process. Participate in bimonthly IDG meetings. Provide service resources as requested. Provide case management to enhance the patient’s quality of life and counsel family members as they adjust to end-of-life care planning. Coordination of care with hospice interdisciplinary team members. Assist family with end-of-life plans such as advanced directives and funeral arrangements. Oversee the volunteer program including recruiting volunteers, conducting all new hire trainings including orientations and tracking hours and duties Communicate need for any volunteer background checks with Human Resources Assist with Federal or State reporting that needs to be done Documentation in EMR as appropriate.SCHEDULE: Part-time hours, approximately 20 hours per week, with potential for full-time hours in the future. Monday, Wednesday, Friday from 8:00 AM - 2:30 PM.
BASIC QUALIFICATIONS FOR THIS POSITION
EDUCATION:
Master’s Degree in Social Work from an institution accredited by the Council on Social Work EducationCERTIFICATION/LICENSURE:
Valid MN MSW licensureEXPERIENCE:
Skilled Nursing Facility, Home Health, Hospice, Hospital, or Clinic experience of at least one year, preferred. Proven ability to work within an Interdisciplinary team. Excellent communication skills: verbal and written. Understanding of the hospice philosophy. Comfortable dealing with suffering, death and dying patients and their family members.KNOWLEDGE, SKILLS, ABILITIES:
Ability to demonstrate and practice the principles of proper body mechanics for all physical activities Ability to practice standards and facility infection control procedures Knowledge of and ability to use position related equipment properly and safely Basic computer skills and the ability to become proficient in company software and data systems Ability to work independently Proven ability to work within an interdisciplinary team setting. Excellent communication skills both verbal and written. Understanding of the hospice philosophy and issues of death/dyingOTHER REQUIREMENTS:
Complete all position related training Obtain and maintain required licenses Ability to travel, valid care insurance, access to personal vehicle, valid driver’s license and good driving record
WHAT WE HAVE TO OFFER YOU:
