Position Profile The hospice social worker provides psychosocial support to patients and families by providing skilled social work intervention to include, but not limited to providing resources, referrals, education, supportive counseling, discharge planning and assistance with final arrangements. The hospice social worker is part of an interdisciplinary team that supports people at critical junctures in their lives, always making a difference. Care delivery primarily takes place in the patient's home which is typically within an assisted living facility.
