Are you experienced, skilled, and passionate about making a meaningful difference? We're seeking a dedicated Hospital and Primary Care Link Worker to engage and assess our service users, ensuring they receive timely, personalised interventions that cater to their unique needs. For this role you will work as part of a multi-disciplinary team providing a wide range of treatment interventions and programmes.

 

Location: Sites throughout East Sussex

Hours: Full-time (37.5 hours per week)

Salary: £27,449.53 - £31,529.41 dependent on experience (based on full time working hours, pro rata for part time hours)

Contract Type: Fixed Term until 31st March 2026

Responsibilities About the Role:

 

Carry out triage and assessments Provide evidence based harm reduction services, including needle exchangeAssist and carry out appropriate screening processes including urine and saliva tests.Provide support for home-based treatments and provide education and support to patients, their families and significant others.Work with partnership organisations to develop and sustain effective comprehensive treatment pathways and broaden the client’s recovery plan accordingly.Attend Hospital and GP multi-disciplinary meetings and contribute to patient specific care plans to improve health and risk of hospital attendance.Ensure that all safeguarding issues, including child protection and protection of vulnerable adults, are dealt with in accordance to CGL policies and procedures, and local and national statutory requirements.Present a professional image and actively promote the service at all times to service users, other stakeholders, partnership organisations and the general public.To assess clients needs as required using evidenced based tools and making onward referrals to specialist agencies, where necessary.

 What we are looking for:

Experience of Drug and Alcohol Treatment services or other similar background.Experience in Alcohol Liaison Work.Excel in both verbal and written communication, with proficient IT skills.Demonstrate proactive qualities, working independently with excellent time management and prioritisation skills.Hold a strong understanding of mental health interventions, services, and best practices.Thrive as a supportive team player with exceptional interpersonal skills, capable of collaborating with a diverse range of professionals, agencies, and internal/external stakeholders.Exhibit the ability to navigate change effectively, always prioritising the needs of service users.Recognise the significance of information governance processes and commit to adhering to all necessary safeguards.Pursue continuous learning opportunities to enhance your professional knowledge and skills, while also contributing to the growth and development of your colleagues.Ability to drive and hold business insurance.

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
