Job Title: Hospital Pharmacy IV Room Pharmacist Key Responsibilities: + Sterile Compounding: + Prepare and dispense sterile medications, including IV solutions, antibiotics, and chemotherapy treatments. + Ensure adherence to strict protocols for aseptic technique to prevent contamination during preparation. + Perform calculations and verify the correct dosage and concentration of medications to be compounded. + Medication Management: + Review physician orders to ensure appropriateness of IV medication therapy. + Adjust medications based on patient-specific factors, such as age, weight, renal or hepatic function. + Monitor the stability and compatibility of drug mixtures, including any interactions between drugs. + Quality Assurance: + Conduct checks to confirm the accuracy of the prepared IV medications before they are dispensed. + Ensure compliance with hospital policies and regulatory standards, including USP <797> (for sterile compounding) and USP <800> and other governing bodies like DNV and FDA regulations. + Participate in continuous improvement initiatives for compounding processes, ensuring patient safety and optimal outcomes. + Inventory Management: + Maintain and monitor the inventory of IV medications and sterile compounding supplies. + Order, store, and rotate drugs and supplies according to hospital policies to avoid wastage or expiration. + Patient and Healthcare Team Interaction: + Collaborate with physicians, nurses, and other healthcare professionals to optimize IV medication therapy for patients. + Provide counseling and education to healthcare staff on the proper administration and handling of IV medications. + Review and make recommendations regarding alternative therapies if there are concerns about a medication's safety or efficacy. + Compliance and Safety: + Adhere to hospital infection control and safety protocols to maintain a sterile environment. + Monitor for potential adverse reactions or side effects of IV medications and work with the healthcare team to adjust treatment accordingly. + Ensure proper labeling of all IV preparations, including patient-specific dosing information and instructions for administration. + Training and Development: + Stay up-to-date with the latest advancements in intravenous therapy and compounding techniques. + Create and facilitate an in-service education programs and contribute to training staff in sterile compounding and IV medication management. Qualifications: + Education: Doctor of Pharmacy (Pharm.D.) or similar degree from an accredited pharmacy school. + Licensing/Certification: Current, active pharmacist license in Mississippi. + Experience: Previous experience in hospital pharmacy, especially in sterile compounding or IV room settings, is preferred. + Skills: + Strong knowledge of sterile compounding techniques and safety protocols. + Proficient in interpreting medication orders and clinical information. + Excellent communication skills for effective collaboration with healthcare teams. + Attention to detail and high accuracy in documentation. Work Environment: + Location: Hospital pharmacy with sterile compounding areas and IV preparation rooms. + Schedule: Weekdays and as needed weekends and holidays This position is vital for maintaining the highest standards of patient care in the administration of IV medications, ensuring that each dose is accurately prepared and dispensed in a sterile, safe, and effective manner.
