Always Best Care Denver is seeking an experienced Hospital & Post-Acute Liaison to drive referrals for in-home care and assisted living placement services across hospitals, skilled nursing facilities, and rehab centers.
This is not a facility-based admissions role — it’s an opportunity to own a defined territory of high-value hospital and SNF accounts, build trusted relationships with discharge planners, case managers, and nurses, and become the go-to referral partner for families transitioning from the hospital or rehab setting to home or assisted living

What You’ll Do
Build and maintain strong referral relationships with hospitals, SNFs, and rehab facilities. Gain the trust of case managers, discharge planners, and other key decision-makers. Navigate complex hospital and post-acute environments to identify referral opportunities. Serve as a visible, reliable partner by maintaining consistent in-person presence across accounts. Respond quickly to referrals, qualify prospective families, and coordinate assessments. Manage a portfolio of ~20 hospital and SNF accounts across the Denver metro area. Document outreach and results in CRM tools; report progress and pipeline weekly. Collaborate with the care coordination and operations teams to ensure smooth client transitions.
What We’re Looking For
Clinical background strongly preferred (RN, LPN, PT/OT, LCSW/LSW, or similar). Proven success as a liaison, marketer, or admissions professional in hospital, SNF, IRF, or home health/hospice settings. Strong understanding of hospital discharge processes and referral pathways. Excellent communicator and relationship-builder with high professionalism. Self-motivated and organized; thrives working independently in the field. Valid driver’s license and reliable vehicle (local travel required).
