Ensures compliance with state and federal grant requirements. Essential Functions + Manages an assigned caseload of clients. + Completes eligibility assessments of clients; discusses and develops a care plan to meet the service needs of the client; arranges and coordinates needed services for clients; and provides follow-up with clients as needed. + Maintains Federal, State, and County mandated client records; prepares reports; and writes letters and correspondence. + Develops and manages a monthly and annual budget for assigned clients. + Contacts and advocates with other community agencies on client’s behalf. + Registers clients for the Special Needs Registry, if applicable. + Assists in crisis interventions; provides clients and families with short-term counseling. + Provides information and referrals for the public and community agencies; receives, processes, and tracks referrals. + Develops and maintains client tracking system for active clients; develops and maintains information and referral database. + Complies with each program’s requirements; reviews and implements new and changing program requirements. + Represents divisional interests at community meetings; attends seminars and in-service training classes. + Provides interviews for clients and schedules home visits; arranges and coordinates in-home services for clients as needed. + Tracks all client contacts on behalf of client. + Provides interviews to walk-in clients to assess their needs. + Resolves conflicts with clients and service providers. + Arranges access to medical care for clients. + Maintains internal controls for senior services. Minimum Qualifications + Bachelor’s degree required. + Two (2) years of related experience. + Candidates without a degree should possess an additional four (4) years of related experience. + Fingerprinting required. LICENSES/CERTIFICATES: + May be required to obtain and maintain certification(s) related to a particular program. + Must possess and maintain a valid Florida Driver’s License with any applicable endorsement(s) to drive a County vehicle as provided in CMA 5805. Supplemental information + Salary offers above the minimum pay grade may be considered based on qualifications. + Essential Employees may be required to work during an undeclared or declared emergency.
