POSITION OVERVIEW:

  Under the supervision of the Clinical Supervisor, the Housing/Case Manager will provide case management services to a caseload and provide housing assistance within the facility.

 

   KEY ESSENTIAL FUNCTIONS:

 

  Case Management:

Provide case management services to a caseload of up to 15Manage and develop systems in the facility.Complete all paperwork for charting as required by funding sourceConduct groups periodically or as directed by management

 

Housing Services:

Provide Housing Assistance to clientsComplete Housing Assessments and ReferralsComplete HRA 2010-E’s (CAPS)

 

  ADDITIONAL FUNCTIONS MAY INCLUDE:

Attend management trainings, team meetings, and staff meetings.Provide assistance within the facility where needed.Performs other duties as required by the Clinical Supervisor. Qualifications

EDUCATIONAL / TRAINING REQUIRED:

Either CASAC or Bachelor degree Preferred  GED/High School Diploma or Associates Degree Required CASAC-T required

 

EXPERIENCE REQUIRED / LANGUAGE PREFERENCE:

 

Minimum 2 years’ experience with population in a residential setting or related field

 

COMPUTER SKILLS REQUIRED:

Proficient in Microsoft Office and related programs
