**Responsibilities:**
The Coordinator, LTSS Housing position is responsible for developing approaches to assist individuals in LTSS programs in securing accessible, affordable housing through Federal, state, and local programs. This position will collaborate with federal, state, and regional affordable housing agencies such as Housing of Urban Development (HUD), State Housing Authorities, subsidized housing, public housing authorities, and voucher programs to:
+ Assist participants in person, as needed, to complete required housing paperwork and determine if other Social Determinates of Health items need to be addressed.;
+ Partner with lead trainers to develop and implement training specific to affordable housing services.;
+ Support Case Managers in identifying housing options for LTSS members.;
+ Serve as subject matter expert and organization resource in housing for people served through Long-Term Services and Supports (LTSS) programs.
+ Develop standards and procedures to be used to support staff in the development and implementation of housing resources.
+ Represent CHC in community meetings and workgroups relevant to housing issues of our members and will help secure housing for those members experiencing housing insecurity and homelessness.
**Education/Experience:**
+ Associate’s Degree or equivalent work experience.
+ 3-5 years experience developing housing and/or housing policy at a local, State, or federal level for special populations such as the elderly, people with disabilities, and/or people with mental health challenges to secure accessible, affordable housing through Federal and local programs.
+ Valid Driver’s license required.
+ Ability to leverage knowledge and experience of affordable housing programs for identified populations at a local, State, and federal level to identify and develop housing opportunities.
+ Ability to effectively present information and respond to questions from staff, members, funders, governmental agencies, and the general public.
+ Proficiency with Microsoft Office applications (Excel, Word, PowerPoint, and Outlook).
+ Strong written and verbal communication skills.
+ Must also provide practical training, prepare written reports, and maintain client documentation.
+ Travel required up to 50%
