Housing Coordinator Supervisor - Behavioral Health 615

Sacramento's Adult Full Service Partnership (FSP) and Laura's Law/Assisted Outpatient Treatment (AOT) program serving a total of 240 adults with serious mental illness and may currently be experiencing or be at risk of homelessness. Members referred may qualify for Permanent Supportive Housing through No Place Like Home (NPLH) units.

Full Time |  8:30am - 5:00pm | Monday-Friday 

Wage range $68,640.00 - $70,152.12 – Telecare applies geographic differentials to its pay ranges.  The pay range assigned to this role will be based on the geographic location from which the role is performed.  Starting pay is commensurate with relevant experience above the minimum requirements.  

POSITION SUMMARY
The Housing Specialist Supervisor is responsible for providing members served with housing services, education, and opportunities. Provides direct customer, community support, crisis intervention, and case management services to assigned customers. Responsibilities include assisting members served in negotiating housing options, interfacing with the housing authority, and managing stable . Collaborates with other services and agencies such as Child Protective Services, County Mental Health and Health Services. Completes screenings and determines the appropriate level of services for new customers.

ESSENTIAL FUNCTIONS
• Demonstrates the Telecare mission, purpose, values, and beliefs in everyday language and contact with internal and external stakeholders
• Provides safe, effective, and efficient implementation of direct care in accordance with established policies, procedures, and standards of care
• Establishes and maintains a therapeutic relationship with members served
• Makes appropriate referrals for living environments, therapy, psychiatrist visits, case management, and other services
• Maintains complete and accurate records on interventions and customer behaviors
• Develops and maintains an awareness of community resources and assists members served in appropriate use of community resources
• Assists members served in obtaining appropriate benefits
• Assures that members served know and understand their legal status and rights
• Provides consultation/interpretation to members served and their families regarding issues arising from treatment of members served
• Assists in the development of and ensures compliance with policies and procedures
• Safeguards customer confidentiality
• Provides individual/group education and training
• Participates as a team member and provides input via reporting observations and concerns and asking appropriate questions
• Works cooperatively with Managers and Program Administrators
• Demonstrates good judgment, decision making, and initiative at performing daily tasks
• Demonstrates knowledge and proper use of equipment and supplies within budgetary parameters
• Reports concerns about issues at programs to the Vice President, Human Resources
• Demonstrates a good rapport and cooperative working relationships with all members of the team; responds to co-workers with concern and promotes group morale
• Demonstrates a knowledge of Telecare's customers including members served, families, and governmental agencies in all interactions and conduct
• Acts in a professional manner, always demonstrating respect and understanding of the community and neighborhood when representing Telecare in the community
• Must evade members served in the event of assaultive behavior and pass assault crisis/crisis prevention training
Duties and responsibilities may be added, deleted, and/or changed at the discretion of management.

Position requires transporting Members in own personal vehicle and program van (acceptable driving record and vehicle inspection for safety).  Position requires driving for company business such as trainings, offsite meetings, etc.

QUALIFICATIONS
Required:
• Two (2) years of direct service experience working with individuals with mental illness, preferably in an unlocked community setting
• Bachelor’s or Master’s degree in a related field, as well as housing experience in the community
• Sensitivity to multi-cultural populations and issues
• Must be at least 18 years of age
• Must be CPR, Crisis Prevention Institute (CPI), and First Aid certified on date of employment or within 60 days of employment and maintain current certification throughout employment
• All opportunities at Telecare are contingent upon successful completion and receipt of acceptable results of the applicable post-offer physical examination, 2-step PPD test for tuberculosis, acceptable criminal background clearances, excluded party sanctions, and degree or license verification. Position requires driving, valid driver license, a motor vehicle clearance, and proof of auto insurance is required at time of employment and must be maintained throughout employment. Additional regulatory, contractual, or local requirements may apply.

SKILLS
• Necessary skills include: the ability to work effectively as a member of a multidisciplinary team, a thorough understanding of client rights, an understanding the conservatorship process, and knowledge of basic medical and psychiatric terminology

PHYSICAL DEMANDS
The physical demands here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

The employee is occasionally required to stand, squat, kneel, twist, do simple and power grasping and lift and carry items weighing 25 pounds or less as well as to frequently sit, walk, bend constantly reach. The position requires manual deviation, repetition and dexterity and be exposed Hazardous Exposure (Blood Borne Pathogens, Hospital Waste, Chemical & Infectious). Visual requirements include computers and books exposure.
