In this vital role, you'll work closely with clients to assess their housing needs, coordinate housing placements, and provide ongoing support to help them maintain stable housing. You'll collaborate with our clinical teams, community partners, and the DBH Housing Unit to ensure comprehensive support for our clients' recovery journeys.

Key Responsibilities:

Coordinate and facilitate housing searches, placements, and success coaching for clientsAssess clients' housing needs, strengths, and resources to develop effective support strategiesAdvocate for expanded housing resources and collaborate with community partnersProvide experiential training in self-advocacy skills and resource acquisitionParticipate in clinical team meetings to contribute to client recovery planningMonitor external services to ensure coordinated careMaintain knowledge of community recovery resources and support systemsEnsure protection of client rights and dignityComplete necessary documentation and reporting requirements

Education, Experience, and/or Credential Qualifications:

Bachelor's Degree from an accredited college or university in social work, psychology, nursing or a closely related fieldEquivalent experience may be substituted on the basis of one year for each year of required educational training with a base education of a High School Diploma or GED

Additional Qualifications:

Must complete background check including criminal record, driving record, abuse/neglect and fingerprint checkCurrent driver's license, acceptable driving record and current auto insurance requiredStrong communication and interpersonal skillsAbility to build effective relationships with clients and community partnersProblem-solving skills and ability to work independentlyMust complete all required training at the beginning of employment and annually thereafter

Physical Requirements:

Sedentary work: Exerting up to 10 pounds of force occasionally and/or negligible force frequentlyPosition involves sitting most of the time with occasional walking or standingRepetitive movements of hands, fingers, and arms for typing and/or writing during work shift
