Shift: Monday - Friday 8:00 A.M. - 5:00 P.M.

Key Responsibilities:

Have knowledge of the Clients Rights and ensure the client's rights are not violated.Provide services at times requested, including weekend and evening hours.Participate in clinical team meetings to identify client strengths and needs related to recovery plan.Assess clients for housing needs, strengths, and resources.Arrange and refer for services and resources, advocating to obtain entitled services.Coordinate and facilitate housing search and placement, and coach for success.Have a working knowledge of community twelve-step recovery resources.Monitor services by external providers to ensure communication and coordination.Provide experiential training in using self-advocacy skills and resource acquisitions.Provide information and education in accordance with rehabilitation plan and discharge plan.May participate in community events, media and committees as deemed necessary.Function in accordance with organization's policies and procedures.Participate in and contribute to staff development activities.Participate in all required meetings.Other duties as assigned by leadership.

Education and/or Experience Qualifications:

Bachelor's Degree from an accredited college or university in social work, psychology, nursing or a closely related field.Equivalent experience may be substituted on the basis of one year for each year of required educational training with a base education of a High School Diploma or GED.
