The Transition Coordinator (TC) is an integral member of the Harm Reduction and Bridges to Care (HRBR) care team, a specialized transitional outpatient clinic for addiction care at OHSU. This position collaborates with social workers, physicians, nurse practitioners, physician associates, pharmacists, peer support specialists/certified recovery mentors and other staff to facilitate patient referrals to services across the addiction treatment care continuum and provides follow-up to ensure successful transition. 

Function/Duties of Position

Make referrals to all levels of care by providing clinical information and documentation to support patient transitions of care to appropriate services. May include scheduling intake appointments, arranging follow up appointments, assisting patient with obtaining PCP.

Complete follow up calls to patients to ensure a successful transition to care. Obtains authorization from insurers using multiple insurance websites, phone and automated systems.

Develop strong, effective working relationships with interdepartmental specialists and programs, outside agencies, facilities, payers, and other relevant community resources in order to facilitate disposition solution for patients and families.

Facilitates all necessary arrangements including:Calls, photocopies, faxes, or mails clinical and/or referral information for outpatient services to:Community substance use treatment agenciesHome Health and Hospice agenciesPrimary careOutpatient mental health careTransportation vendorsSNF, ICF, AFH, RCF, and Assisted Living facilitiesShelter operatorsOutside Case ManagersMedicaid CaseworkersInsurance companiesTypes letter of medical necessity, if neededCompile and organize treatment resources that are easily accessible to all Bridge clinic care team members. Required Qualifications A bachelor’s degree in Business Administration, Management, Public Administration, or a field directly related to the position will substitute for two years of the required experience.Five years of experience with responsibility for program or project monitoring and coordination.  The experience must have included program evaluation responsibility. 

Job Related Knowledge, Skills and Abilities (Competencies):

Proficiency with Microsoft Office Software especially Word, Excel, Outlook, Teams, Power-Point. Demonstrated ability to use EMR systems, including familiarity with patient registries and community resources.Ability to be self-directed, and experience managing multiple tasks and timelines, and utilization of excellent organizational skills. Accustomed to frequent interruptions and comfortable with change Demonstrates professional, appropriate, effective, and tactful communication skills, including written, verbal and nonverbal. Includes excellent listening skills. Ability to build and maintain professional relationships with inter-professional team of physicians, social worker, and peers with lived experience in recovery. Proactive acts as patient advocate, responding with empathy and respect to resolve patient and family concerns, and recognizes opportunities for improvement to meeting patient concerns. Preferred Qualifications Experience working in community substance use treatment or community mental health settings. 3+ years’ work experience in healthcare disciplines, including but not limited to care coordination, case management, community mental health, chronic illness, patient education and complex social issues; other relevant experience, includes working with an adult under-served population and work in the Portland area safety-net. Must have familiarity with common outpatient medications and general medical Experience and training in healthcare discipline that provided the opportunity to manage care coordination for various patient populations. Valid OR/WA driver’s license with an acceptable driving history and access to a car on a daily basis for home visits.Working knowledge of medication and treatment for substance use disorders (SUD) Additional Details

This position is 100% grant funded.

This position is hybrid (mixture of remote/virtual interventions and community-based interactions) and may experience frequent interruptions. The candidate will need to effectively prioritize and multi-task, be able/confident with virtual work environment and balance competing demands from clinic staff and patients.
