Acentra seeks a IDD Waiver Assessor Service Support Facilitator to join our growing team.

Job Summary:

The I/DD Waiver Service Support Facilitator is vital to the program's success by conducting initial and annual assessments which are used in determining medical eligibility and individualized budgets for program members. The individualized budgets are crucial for the member's planning team in guiding strategic service purchases for community support. The facilitator will assist in the process of redetermining medical eligibility and calculating individualized annual budgets through initial and annual assessments.

*Position is hybrid. Candidates must have a strong internet connection to be able to work remotely as well as be available to travel to catchment area and Charleston as needed.

 

Job Responsibilities:

Schedule, conduct and appropriately document individual assessments within prescribed timelines.   Enter assessment data into the web system.   Participate in inter-rater activities to ensure consistency in the administration of the assessment instruments and adherence to assessment protocols. Participate in monthly I/DD Waiver staff meetings, quarterly all-staff meetings and other required training and meetings as necessary. Support in overseeing department procedures and participating in quality assurance/improvement initiatives. Conduct educational sessions for family members and agencies, covering policies related to assessments and budgetary matters. Complete satisfaction survey with a focus on achieving positive results from respondent feedback.  Other duties as assigned.
