\nResponsibilities
\n\n Completing Level of Care Eligibility Tool (LOCET) and other applicable assessments.\n Making appropriate referrals to non-OAAS community services.\n Analyzing requestors’ needs and researching available resources to meet those needs.\n Completing intake process for requestors of Louisiana long-term care programs.\n Managing incoming and outgoing phone calls to Waiver and Long Term – Personal Care Services (LT-PCS) requestors and recipients.\n Utilizing CONDUENT and DHH databases to input and update requestors and recipients’ information and track applicants/recipients’ application process for Elderly and Disabled Adults (EDA), Adult Day Healthcare Center (ADHC), Program for All-Inclusive Care for the Elderly (PACE) and LT-PCS programs.\n Handling TTD/TTY (telecommunications for deaf or hard of hearing) assistance calls.\n Participating in appeal hearings.\n Providing consultation and assistance with the application process.\n Participating in Continuous Quality Improvement (CQI) process. \n Performing post-assessment tasks related to LOCET mailing and follow-up for EDA and ADHC requesters and recipients.\n Ensuring that problems are promptly addressed, documented, and rectified on time, with the proper follow-up and professional customer service.\n Providing prompt, professional customer service to ensure that all necessary information is covered and gathered from requestors and recipients in a clear, consistent, and professional manner.\n Participating in departmental “team effort” functions and projects.\n Participating in ongoing departmental and CONDUENT training activities.\n Performing other duties as assigned by management staff.\n\nRequirements
\n\n Have a bachelor’s degree in a human services field.\n Possess a minimum of one year experience in social and/or human services, preferably working with the elderly and disabled adult population.\n Exhibit working knowledge of and experience with the Office of Aging and Adult Services (OAAS), Bureau of Health Services Financing (BHSF), Department of Health and Hospitals (DHH), Medicaid, Office of Citizens with Developmental Disabilities (OCDD), and/or Home and community-based waiver services.\n Know about Medical and developmental disability terminology.\n Able to maintain strict confidentiality, work independently and autonomously, and remain flexible and composed under pressure.\n Have experience with social assessments and evaluations.\n Exhibit strong written and verbal communication skills.\n Possess effective task and project organization abilities. \n Be proficient in word processing and relevant computer applications. \n Have experience in customer relations and service. \n Demonstrate leadership and motivation.\n Show willingness and ability to travel. \n\nFlexible Working

For this position, actual salaries will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. In addition to base pay, this position, based on business need, may be eligible for a bonus or incentive. In addition, Conduent provides a variety of benefits to employees including health insurance coverage, voluntary dental and vision programs, life and disability insurance, a retirement savings plan, paid holidays, and paid time off (PTO) or vacation or sick time. The approximate compensation for this position is $15.63 per hour.
