Job Overview:
A TriHealth Clinical Pharmacist is responsible for providing optimal pharmaceutical care to all patients through distributive and clinical functions. The Clinical Pharmacist collaborates with other healthcare providers to ensure the appropriateness of all elements of pharmaceutical care and independently manages the department’s clinical and consultative services in accordance with policies and procedures. The Clinical Pharmacist supervises the work of supportive team members to ensure the accuracy and efficiency of distributive services. Additional responsibilities include serving as a drug information resource for TriHealth providers, direct patient interaction, precepting pharmacy residents and/or student pharmacists, and contributing to department projects or initiatives in collaboration with Pharmacy Leadership.
Job Requirements:
Bachelor's Degree in Pharmacy Registered Pharmacist with Ohio Board of Pharmacy Proficiency in prescription formulas and mathematics for calculating dosages and quantities Working knowledge of all Ohio Board of Pharmacy regulations 1-2 years of experience in PharmacyJob Responsibilities:
Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm, professional environment. Establish effective collaborative working relationships and open communication with pharmacy leadership, physicians, nurses, pharmacists, and technicians. Contribute to direct patient care through careful review of patient information and provision of appropriate pharmaceutical recommendations. Promote the correct and appropriate use of medications by maximizing clinical effects, using the most effective treatment, minimizing treatment-induced adverse events, and reducing pharmacological treatment costs. Communicate findings and recommendations to other healthcare professionals responsible for the patient’s care, ensuring issues are resolved before dispensing. Provide education and counseling to patients about drug therapies, and provide concise, timely responses. Ensure the 7 patient “rights” are respected: right patient, right dose, right route, right time, right drug with the right information and documentation. Report medication errors and assist in assessing risks and strategies to manage risks. Ensure compliance with regulatory requirements and organizational policies and procedures. Provide a consistent process of patient care to ensure the safety of the patient's medication use. Assist in protocol development, implementation, and optimization. Undertake projects that will improve the quality of patient care and promote cost-effective use of medications. Provide drug distribution skills including verification, order entry, clarification as needed, accuracy of checking, technician supervision, and timely delivery. Utilize clinical knowledge and experience to resolve health problems through rational use of medications. Manage drug inventory by adjusting par levels, avoiding stock-outs, and decreasing waste. Communicate with Pharmacy Buyers to ensure supply matches demand. Ensure complete, accurate, and timely documentation. Identify opportunities and solutions for pharmacy process improvements. Apply known solutions to predictable problems. Assume personal responsibility for professional development, particularly in ongoing clinical education, and develop clinical pharmacy programs according to policies and regulations. Participate in relevant committees. Assist in the orientation and training of new hires, and precept pharmacy students and residents.Other Job-Related Information:
