RESPONSIBILITIES:
 

Care for individual based on personalized Plan of Care Informs the Director of Nursing and any other appropriate personnel of changes in the client’s conditions and needsCounsels client and family through care process while updating family on a regular basisAssures proper maintenance of clinical records in compliance with local, state, and federal lawsParticipates in agency performance improvement activities and continuously staying informed about changes in the field of nursing and home health care

REQUIREMENTS:
 

Current unrestricted RN license in the state of PennsylvaniaExperience with venipunctureExperience with chronic disease education and managementOASIS not requiredReliable car/auto insuranceCPR Certified-American Heart Association/Red CrossCurrent Physical & negative TB Test/Chest X-RayClean background and criminal recordUnderstands regulatory compliance with state and federal regulationsAdhere to HIPAA and maintain client confidentiality
