Infusion Pharmacist Responsibilities:

Reviews and verifies all prescription medication orders received from the prescriber or provider’s agent and evaluates appropriateness and completeness of prescribed therapies in a home setting.Assesses the patient at the initiation of IV drug therapy, develops a patient care plan and drug monitoring regimen and monitors the patient according to the care plan throughout the course of therapy.Ensures drug, related necessary supplies and all patient collateral (education materials) are accurately provided to our patientsFacilitates communication with the patient/caregiver, physician, provider, home health nursing staff, and other members of the health care team regarding patient status, response to therapy and clinical objectives to ensure optimal clinical outcomes.When required, must be able to compound intravenous and specialty pharmaceuticals products using aseptic techniques according to established Florida Board of Pharmacy, federal agencies, USP
