Inpatient Pediatric Medical Social Worker Social Services (Social Work, Case Mgmt) Johnston, Iowa Apply Description We are seeking a compassionate and skilled Pediatric Inpatient Social Worker to join our multidisciplinary healthcare team. This role provides critical psychosocial support to pediatric patients and their families in an inpatient setting. The Social Worker will assess patient needs, develop care plans, coordinate services, and provide crisis intervention while advocating for the well-being of children with complex medical conditions.What You'll Do + Assist families in accessing resources and advocate for their cultural and individual needs. + Collaborate with the care team to develop treatment plans and support families’ emotional needs. + Continuously assess patient needs and coordinate complex discharge planning. + Provide palliative and end-of-life support in collaboration with medical teams and stakeholders. + Manage discharge planning and referrals to ensure timely transitions. + Act as the primary liaison with the Department of Human Services. + Utilize therapeutic interventions (Motivational Interviewing, ACT, Solution-Focused) to support families. + Stay informed on community and ChildServe resources. What You'll Need + Master’s degree in social work from an accredited School of Social Work + Licensed to practice social work in Iowa. + Experience practicing as a Medical Social Worker in a pediatric setting + Experience working in an interdisciplinary skilled healthcare setting + Ability to communicate in English effectively for understanding, in writing and verbally.
