Deliverables/Principal Results Expected:
• Explain and sign admission paperwork, process and copy driver’s license/insurance information, create/start client treatment file.
• Contact Probation/Parole/Employer if needed for client and assist in getting letter for admission/discharge
• Facilitate admission/discharge process by communicating with all departments as needed.
• Maintain paperwork/notes/application statuses as needed for client stay/discharge needs per funding source requirements
• Conduct level of care assessments on all clients.
• Complete aftercare goals with detox only clients, set up aftercare and complete discharge summaries.
• Other duties as assigned.
Licensure, Education, & Experience:
High school diploma or equivalent Associates or bachelors degree in human services related field preferred.Total Rewards:
