**Job Description** The Intake Coordinator serves as a resource for patients seeking medical services. Provides clients with verbal and written information as well as assisting in all pre-admission services leading up to and following medical services. **Qualifications** High School graduate. 1 year office clerical experience. **Responsibilities** 1. Provides information to prospective patients giving tours, printed materials, answering inquiries, general overview, costs of treatments, insurance coverage, length of stay, payment expectations, rules and guidelines as well as available amenities. 2. Conducts and documents phone assessments of potential patients which covers but is not limited to demographic information as well as a brief history of substance use, mental health disorders, previous treatments, current medications and any physical issues. 3. Assists the Outreach Liaison with presentations and information provided to the larger community of consumers or providers. 4. Maintains contact with prospective patients who may need continued assurances and encouragement to engage in treatment. 5. Maintains lists of prospective patients contact phone numbers and addresses to provide information, progress, assurance and encouragement. 6. Schedules admission, meets and escorts patients and family members upon arrival to explain procedures and admission process. 7. Ensures all documents are completed and complies with applicable regulations including release forms, family contact information and other documents per HIPAA and Mount Sinai policies. 8. Assists with discharge and aftercare planning as well as outpatient and inpatient providers and facilities. 9. The salary range for the role is $64526.72 - $73937.44 Annually. Actual salaries depend on a variety of factors, including experience, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
