Position Overview:
Serves as the initial contact for prospective enrollees and their representatives to provide information about Capital Health PACE.Obtains demographic, medical, insurance, financial, and referral information to determine eligibility and interest. Refers individuals to other appropriate resources when necessary.Establishes and maintains positive relationships with prospects. Visits homes and other settings. Reviews enrollment documentation and ensures potential participants understand program services. Acts as liaison between prospects and staff as necessary for communication, scheduling, care plan development, and enrollment.Coordinates and continually monitors each component of enrollment process to ensure all deadlines are met and initiates changes as necessary. Schedules enrollment assessment process and maintains documentation in various computer programs for tracking and reporting purposes.Assists with Medicaid financial application process. Reviews Medicaid eligibility, provides administrative support, monitors approval timeframe, and performs other tasks to facilitate a smooth application and certification process.Communicates regularly with all Capital Health PACE members of the Interdisciplinary Team to share key information. Coordinates tasks and provides support for the enrollment process as needed.Establishes and maintains positive working relationships with referral sources to regularly discuss possible program candidates and ensure a clear understanding of PACE program as a vital community resource. Maintains similar relationships with Medicaid and other industry contacts.Assists with marketing activities as needed, including representing Capital Health PACE in community presentations, special events, health fairs, and trade shows.Performs other duties as assigned.MINIMUM REQUIREMENTSEducation:Associate's degree in related field. Bachelor's degree in healthcare or human services field preferred.Experience:One year of experience in case or care management, benefits administration, marketing, or sales experience in a healthcare.Other Credentials:Drivers LicenseKnowledge and Skills:Excellent organizational skills. Ability to perform multiple duties and functions related to daily operations and maintain excellent customer service skills. Ability to perform frequent detailed tasks and provide immediate service with frequent interruptions. Ability to change and be flexible with work priorities. Strong problem-solving skills.Special Training:Comprehensive to expert proficiency with Microsoft product suite (MS Word, Excel, Power Point). Basic knowledge and experience with email and calendaring system.
