Seeking a dedicated Intake Nurse to join our large hospice organization. Responsibilities include coordinating referrals, verifying and entering physician's orders, determining the appropriate level of care for patients, reviewing documentation for facility placement, and coordinating orders with pharmacists. The role demands close collaboration with referring hospitals, physicians, skilled nursing facilities, patients, and their families.
Hard Skills
+ LPN or RN license in the State of Utah
+ 1 year experience in a skilled nursing facility
+ BLS certification
Responsibilities
+ Coordinate referrals
+ Verify and enter physician's orders
+ Identify the level of care required by patients
+ Review documentation to determine facility placement
+ Coordinate orders with pharmacists
+ Work closely with referring hospitals, physicians, skilled nursing facilities, patients, and families
Additional Skills & Qualifications
+ Experience with MatrixCare or Point of Care EMR
Work Environment
The position follows a M-F schedule between hours of 8am-7pm and will rotate and work a weekend every 6-8 weeks! The role also requires being on call every 6-8 weeks. The dress code is business casual, and the work setting is an office environment with a team of 32 individuals and 9 on the immediate team.

The pay range for this position is $32.00 - $32.00
