Description
TheIntake Specialist, TGNCNB Hubwill assist with screening, intake sessions, and recruitment for the GMHC & Callen-Lorde/CDC Initiative as well as for the entire TGNCNB Hub dedicated to Transgender, Gender Non-Conforming and Non-Binary community members. The Intake Specialist will build an authentic rapport with clients, act as a role model, and share personal strengths and skills while promoting hope and recovery. The Intake Specialist must be knowledgeable of community resources available to clients internally and externally. Duties may also include administering surveys, data gathering, maintaining participant databases, assisting with event organizing, and performing various administrative tasks.
Essential Job Functions
The following duties are mandatory requirements of the job:
+ Conduct screening and intake sessions for potential participants for the GMHC & Callen-Lorde initiative within the TGNCNB Hub. They will also support with program intake and screenings for other initiatives within the Hub.
+ Schedule intake appointments with potential new participants.
+ Follow-up on missed appointments, and scheduling and rescheduling appointments.
+ Conduct outreach, recruit, & engage TGNCNB community members
+ Health promotion, health education and risk reduction counseling/promotion.
+ Assist the Coordinator and Peer to provide check-ins, linkages, referrals, care coordination, case closure, and the completion of the proper documentation for all HIV Navigation Services (HNS)
+ Motivational interviewing and goal setting with clients
+ Coordinate linkage to services with internal and external providers
+ Routine communication with all members of Prevention Programs.
+ Maintain all documentation of services provided via TREAT/AIRS and other data entry system provided by Callen-Lorde
+ Attend meetings and trainings deemed appropriate and related to the scope of service
+ Adhere to workplace policies and procedures, including confidentiality, documentation, channels of communication, workplace culture, dress code, and conflict resolution.

+ Assists with special projects and performs other related duties as assigned by supervisor.
Education and Certification
+ H.S. Diploma, GED, or HSE required AND experience in implementing Community-Based HIV/AIDS Prevention Programs (HIV Testing, Outreach, Benefits Navigation, Programming for TGNCNB communities, etc.)
Special Skills and Knowledge
In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:
+ Ability to make data entries into the computer databases
+ Thorough knowledge of social, health/medical and mental health and wellness systems, including treatment center programs, and the criminal justice system
+ Good verbal, written, computer, communication and interpersonal skills
+ Use of computer software --Microsoft Office Suite including Word and Outlook calendaring, and Excel
+ Bilingual in English and Spanish helpful
+ Ability to multi-task in a fast paced, outcomes-oriented environment
Salary Description
$25 per hour -20-25 hours per week
