Intake Specialist (SEP)

Position Summary:
The Intake Specialist serves as a member of the Treatment team of WestCare Tennessee and will work collaboratively with the Project Director, Treatment Counselor, Case Manager, Supported Employment Coach and Peer Support Specialist. This role primarily conducts the intake process to help determine the eligibility of programs and/or services for the participants at WestCare Tennessee. This position will serve individuals from Cocke and Jefferson County Tennessee.
Essential Job Functions:
Embraces and embodies the mission, vision, guiding principles, and goals of WestCare Tennessee. Provide a professional first impression appearance to participants/visitors as they enter the building. Administer prescreening and agency approved program needs assessments to help determine the appropriate level of service needed by participants seeking treatment. Upholding strict confidentiality of individual information. Assist participants in completing introduction packets with the required preprogram components. Obtain all necessary documentation and verification forms to determine program eligibility. Complete documentation of services on provided forms and inputting work activities in required data tracking software platforms promptly and according to deadlines. Perform front office tasks which include answering incoming calls and documenting messages from participants and visitors. Document inquirer’s calls, written messages, and electronic correspondence regarding programs, agency information, events, and appointments then deliver message to Project Director or appropriate staff. Make outgoing and/or follow up appointment calls as directed by program staff. Coordinate with program staff to conduct orientation of participants enrolling in programs and make sure participants are provided all necessary program materials to fully participate in WestCare Tennessee programs. Attend necessary monthly integrated staff team meetings. Maintain strong professional relationship boundaries with individuals served. Maintain strong professional relationship boundaries with organizations/ community agencies who refer individuals to WestCare Tennessee. Submit weekly report to Project Director. Work closely with Project Director, Research Assistant, and WestCare Foundation Evaluation team regarding the collection of data and reporting of program outcomes. Attend in-person and virtual program conferences and meetings, community meetings, and events as deemed necessary. Perform additional tasks that support a productive office workflow as deemed necessary by Project Director. Will engage and complete the required WestCare Tennessee eLearning, and any other program related trainings.
