Unlock your potential! **Job Summary and Qualifications** + Conducts individual, group and family psychoeducational and process groups. · Develops treatment plans and attends treatment planning meetings and reviews on an ongoing basis. · Assesses environment and patient’s safety status and reports/responds to actual and potential safety issues or changes in the patient's condition. · Identifies risk factors, screening criteria and interventions for patients with special precautions (e.g., suicide, homicide, falls, elopement). · Completes assessments, group notes and all other documentation promptly and in compliance with requirements, as appropriate to scope. · Collaborates with the multidisciplinary team to assess and coordinate patient’s discharge planning needs and readiness for discharge. · Conducts post-discharge follow-up as indicated. · Evaluates re-admissions for gaps in discharge planning processes. Formulates a plan and implements new strategies. · Develops and maintains a community resource guide including access to public assistance/alternatives. · Facilitates and participates in Community Education, Activities and Support Groups, as appropriate to scope. · Contributes to ensuring a safe, structured milieu with evidence-based treatment modalities/resources. · Identifies cultural considerations, gender identity and preferred language for patients and family. · Demonstrates appropriate application of age/developmental stage specific standards, policies and procedures, and guidelines in assessing and treating patients to ensure patient safety. · Maintains active and timely communication with patients, families, multidisciplinary team and continuum of care providers. · Maintains professionalism and uses good communication skills in interacting with peers, staff, patients, families and in resolving conflicts. · Demonstrates understanding of and ability to maintain clear professional boundaries with peers, patients and families. · Understands state commitment processes, abuse reporting requirements, duty to warn and guardianship processes. · Demonstrates knowledge and role in hospital codes. · Serves as an educational resource to patients, families, interdisciplinary team members, payer representatives, and other parties as appropriate regarding behavioral services. · Reviews, coaches, and assists with training of new staff/interns as assigned within approved practice and program guidelines. · Participates in developing department goals/objectives and clinical programming. · Performs utilization review as assigned. · Assists with Process and Quality Improvement initiatives. · Performs other duties as assigned. · Practices and adheres to the “Code of Conduct” and “Mission and Value Statement.” **What qualifications you will need:** + Basic Cardiac Life Support must be obtained within 30 days of employment start date + Nonviolent Crisis Intervention must be obtained within 30 days of employment start date + Masters Degree Committed to the care and improvement of human life, Medical City Healthcare aims to deliver excellence - providing the highest quality, cost-effective healthcare in the communities it serves.
