Our Adult Residential Programs serve adults with chemical dependency and/or are living with a mental illness at one of our four different locations in Albany County. The goal of all our programs is to help the residents manage their diagnosis and build the skills necessary to live successfully in the community.
Holt House- Adults with both mental illness and developmental disabilities that need 24/7 support
Apartment Program- Adults who live in one- or two-bedroom apartments and are ready for less structure and more independence
Cohoes Residence- Adults who have a mental health diagnosis and need 24/7 support to help control their symptoms
Recovery Residence- Men who have both a mental illness and an addiction that need 24/7 support.
Position Summary:The Intake Utilization Coordinator will be responsible for managing housing intakes, follow-ups, discharges, and other essential intake-related activities for clients. This position requires ensuring clients have the correct documentation to secure active housing waitlist status, maintaining accurate client records, and collaborating with various community organizations and external agencies.
Essential Duties and Responsibilities:
Housing Intake and Follow-Up:
+ Conduct and coordinate housing intakes, ensuring all necessary documentation is completed accurately and in a timely manner. Provide ongoing support to clients throughout the housing process, including verifying active Medicaid status and assessing financial capacity to pay rent
+ Housing Waitlist Management: Maintain and update the housing waitlist regularly, ensuring all clients are appropriately prioritized based on eligibility and need. Ensure the SPOA housing waitlist status is up to date and accurately reflects client placements
+ Intake Logistics: Coordinate in-person or Web-based intake appointments, which may require traveling to specific locations such as hospitals, jails, and other facilities
+ Receiving and Reviewing Client Referrals: Receive and review client referrals to ensure they meet the agency’s criteria for Equinox Services/OMH. Ensure all referrals are completed accurately and in accordance with agency standards
+ Intake Logistics: Coordinate in-person or Zoom-based intake appointments, which may require traveling to specific locations such as hospitals, jails, and other facilities
+ Discharge and Coordination Meetings: Actively participate in discharge meetings, AOT (Assisted Outpatient Treatment) coordination, PSCC (Program Support and Coordinated Care) meetings, and county Single Point of Access (SPOA) meetings
Compliance
+ Occupancy and Benefits Monitoring: Review and maintain up-to-date occupancy records for clients. Monitor and track benefits, including Medicaid, to ensure clients receive proper health insurance coverage. Ensure required documentation is submitted through the system, while maintaining eligibility for benefits
+ Community Engagement: Attend and represent the agency at community meetings related to housing. Build and maintain positive relationships with community organizations, housing landlords, and other stakeholders
+ Apartment Acquisition and Certification: Secure suitable apartments for clients, ensuring compliance with the Office of Mental Health (OMH) certification process. Complete all necessary OMH paperwork and ensure apartments are properly certified.
+ Attend bi-weekly SPOA (Single Point of Access) meetings, providing updates and ensuring coordination of housing services for clients
+ Collaboration with External Agencies: Communicate and collaborate with external agencies regarding intake processes, housing placement, and client services
+ Tracking and Reporting: Maintain internal tracking reports, including admissions, discharges, and residential waitlist updates. Use the Evolv Medical Record System to track client documentation, including entering client demographics, residential program admissions, discharges, and transfers
+ Compliance with Regulations: Ensure client records follow SSA (Social Security Administration) and HIPAA (Health Insurance Portability and Accountability Act) regulations to maintain privacy and confidentiality
+ Director Meetings: Meet with the Director on a as scheduled basis to communicate any questions or concerns related to the intake process, client rent, insurance, and to ensure clients have sufficient funds to afford rent
+ Agency Representation: Represent and reflect Equinox Inc.'s mission, values, and philosophy in all interactions and tasks.
+ Other Duties as Assigned
Requirements
Physical Requirements:
+ Ability to sit for extended periods
+ Ability to lift at least 20lbs
+ Ability to safely operate a vehicle
Minimum Qualifications:
+ Associate’s Degree in a related field with at least 4 years of relevant experience OR Bachelor’s Degree in related field with at least 2 years of relevant experience
+ Must have a clean, valid New York State Driver’s License AND have access to reliable transportation, as needed.
+ Experience in housing, social services, or a related field, particularly in intake, coordination, or utilization management.
+ Knowledge of housing waitlist management, and client intake procedures.
+ Understanding of SSA, HIPAA regulations, and confidentiality standards.
+ Ability to perform client assessments and screenings, including determining active Medicaid and income sources.
+ Strong communication skills, both written and verbal, to effectively engage with clients, agencies, and community partners.
+ Ability to work independently and handle multiple tasks with attention to detail.
Preferred Qualifications:
+ Bachelor’s degree in Social Work or Human Services.
+ Knowledge of OMH certification processes preferred.
+ Familiarity with Evolv Medical Record System and Integrated Reporting Systems (CAIRS) preferred.
Schedule:Monday through Friday.
