Status: Full-Time, permanent position (40 hours)
Standard working hours: 8 am to 5 pm EST, M-F
Location: Remote but will conduct home visit in LTSS/Medicaid population members' homes. Need to reside in Tidewater, VA (Norfolk, Virginia Beach, Suffolk, Portsmouth and Chesapeake, VA).
As an Integrated Care Manager with Sentara, you get to work together in an intensely collaborative way with the patients, family, and members of the medical team to provide patients with the resources they need and help prevent them from readmitting into the hospital. This position's primary duty is customizing what is best for the patients. That would include helping with things like home health, long-term care placement, home medical equipment, homeless and community resources. Job qualifications:
Min 3 years Nursing experience REQUIRED
Registered Nurse/RN License in Virginia/Multi-state/Compact REQUIRED
For Integrated Care Management departments, specialty certification REQUIRED within one year of eligibility (ACM, CCM, CCCTM or RN-BC). For other service lines, certification based on specialty area required within one year of eligibility.
Preferred qualifications:
3 years Case Management experience preferred.
N-2YR - RN-Associate's Degree
N-4YR - RN-Bachelor's Level Degree
N-3YR - RN-Diploma (Non-degree)
N-DN - RN-Doctorate Level Degree
N-6YR - RN-Master's Level Degree
