Job Highlights:

Must be a NC Certified Peer Support Specialist OR NC Certified Community Health Worker.

This Opportunity:The Integrated Health Coordinator will provide support to individuals with high utilization of Emergency Department services. The position will perform a variety of activities that address unmet physical health, behavioral health, and/or pharmacy and health-related resource needs.

What You'll Do:

Connect individuals to their current primary care provider, coordinate annual visits for preventative care, and assist with ad hoc scheduling for illness or other primary health concerns.  If there is no current primary care provider, support individuals by making a referral to primary care services

Follow-up with each individual during an emergency department visit.  If unable to meet in person during the emergency visit, the Integrated Health Coordinator, will make best effort to engage with the member in the community within 24 hours and make efforts to complete a second visit within seven days.

Coordinate and support individuals attending follow-up care after every emergency department visit and/or inpatient discharge

Utilize Person-Centered Thinking strategies to develop rapport with individuals and assist them with maintaining engagement with their service providers

Engage in various health promotion activities that promote self-management and wellness

Gather historical records information through NC HealthConnex, electronic health records, and via manual records requests and share the information with the assigned treatment providers and Care Management providers, as applicable

Follow the individual’s Crisis Plan, as identified by the Tailored Care Management Care Plan/ISP or Provider’s PCP.  For instances where there is not a Crisis Plan in place, coordinate linkage to the appropriate team and assist in developing a Crisis Plan.

Link individuals to appropriate community-based services and resources as well as monitor engagement with each service

Maintain trainings as required

Demonstrate knowledge of emergency procedures and assist in crisis situations

Demonstrate knowledge of and comply with all agency policies and procedures

Complete all other relevant responsibilities assigned by the supervisor

Driving and travel is required in and around the Wake County area to ensure adequate support to assigned cases

Education We're Looking For:High School Diploma (Required)

Certifications We're Looking For:Certified Community Health Worker (CCHW) - North Carolina Community Health Worker Association, Certified Peer Support Specialist (CPSS) - State Division of Mental Health, Developmental Disabilities and Substance Abuse Services (USA), Drivers License (Valid) - USA

Experience We're Looking For:Experience working as a Certified Community Health Worker., Experience working with individuals in a primary care, hospital, or other health care setting.

Schedule:Schedule to be determined, based on caseload needs.
