Intensive Outpatient Group Facilitator
Job Summary : Master’s/Licensed Intensive Outpatient Group Facilitator (IOT) provides individual, family, and group therapy services to assist consumers in addressing mental, behavioral, or emotional issues that may be impacting their daily lives and relationships with others. Utilizing Wellness and Recovery principles, the IOT group facilitator empowers consumers in achieving time-limited goals identified in their Person-Centered Treatment Plan. The IOT group facilitator functions as an active member of the consumer’s interdisciplinary treatment team with the common focus of guiding the consumer through the process of recovery, resulting in the consumer achieving self-direction, a redefinition of self, and transcendence from shame and stigma associated with mental illness. The services that the group facilitator provides are under the supervision of an HSPP and are driven by the treatment team of the consumer. Group facilitators utilize the following models and approaches to guide the consumer through the process of recovery: + Person-Centered Treatment Planning (PCTP); + Motivational Interviewing (MI); + Solution-Focused Brief Therapy (SFBT); + The Wellness and Recovery Model and Approach; + And Other Best Practice Modalities. Maintaining regular and reliable attendance is necessary in order to carry out these Essential Functions. Essential functions of this job may include, but may not be limited to: + Facilitate groups in the Intensive Outpatient Treatment program 3 days per week for 3 hours each session. May also facilitate aftercare groups or help to cover other groups as needed; + Ability to follow all standards of facilitating the Intensive Outpatient Group in order to maintain compliance with regulations; + Ability to engage with consumers, their families and staff to develop relationships that are conducive to promoting confidence, independence, and self reliance; + Provides individual, family, and group treatment modalities to persons from diverse cultural backgrounds, including the utilization of age-appropriate techniques for children and adults; + Utilizes a holistic approach to strengthen the family system necessary to meeting the treatment goals of the consumer; + In the appropriate therapeutic setting, meets with individuals and groups, including children and their families from diverse cultural backgrounds, to assist them in assessing strengths and identifying barriers and developing realistic plans to address them. + Document services provided via electronic and paper records outlining medical necessity, consistent with internal and external standards; + Administer the CANS/ANSA instrument as necessary to plan treatment, monitor clinical outcomes, and improve the quality of services; + Participates as a member of the consumer’s interdisciplinary treatment team during staffing times and case conferences as necessary in order to oversee the coordination of the consumer’s treatment care; + Interact and collaborate with a variety of referral agencies such as criminal justice providers, DCS, and prescribers via phone, in person, or by sending monthly reports in a timely manner. + Develop, monitor, and implement the consumer’s Person-Centered Treatment Plan and review the plan as necessary and clinically appropriate to reflect the consumer’s identified needs and achievements; + Provide consultation with community agencies and families to maintain coordination of care during the treatment process; + Will demonstrate knowledge and ability to manage time productively and efficiently by managing and meeting Direct Service time expectations, monitoring case load reports, managing expenses and completing weekly schedules; + Ability to recognize signs of abuse/neglect, develop an understanding of reporting procedures, and best practice intervention; + Ability to recognize signs and symptoms of substance abuse, how to refer for further assessment or medical care, and best practice clinical interventions; + Ability to identify and assess consumer risk factors, ability to follow best practice clinical interventions, and knowing how to refer for further assessment when necessary; and, + Other duties as assigned by clinical management. Additional Responsibilities: + Participates in training programs including the teaching of diagnosis and treatment methods conducted for the benefit of the community, and may act as a consultant for other community agencies. + May provide additional services of Case Management, Skills Training and Development, and Crisis Intervention to consumers in need; + Will become a Recovery Works provider and must be able to see DCS consumers Policies and Procedures: Ability to follow corporate guidelines related to personnel policies and procedures and compliance with internal and external standards including: + Will support and demonstrate through his/her actions the philosophy of Cummins Behavioral Health Systems, Inc., as well as its corporate mission, vision, and values; + Will abide by Cummins’ policies and procedures, and will meet all documentation requirements; + Comply with internal and external reporting and compliancy requirements: + As applicable, will implement DMHA promulgated rules governing residential facilities; + Will ensure compliance with Joint Commission, DMHA, OMPP, OSHA, and other internal/external standards; and + Adherence to and promotion of the Corporate Compliance Program at Cum mins Behavioral Health Systems, Inc Education and/or Experience: + Masters degree in behavioral health, social work, counseling or other related field, and be eligible for license in the state of Indiana as an LCSW, LMHC, and/or LMFT (candidates dual licensed with the LAC/LCAC preferred); + Two years postgraduate supervised clinical experience preferred. + Group facilitation experience and treating substance use disorder preferred. Knowledge, Skills & Abilities: + Must possess competent interpersonal and written/verbal communication skills, including clinical documentation skills; + Must be able to document services with an electronic clinical record, clearly indicating an understanding of the consumer’s medical necessity for each service provided; + Ability to plan and implement Person-Centered Treatment Planning (PCTP) for consumers throughout the clinical therapy process; + Must be knowledgeable and be able to implement the Wellness and Recovery Model and approach; + Must be knowledgeable and conceptually understand the Life Skills Specialist Service Delivery Model for Cummins Behavioral Health Systems, Inc. necessary to forming a collaborative relationship with interdisciplinary treatment team in planning consumers’ treatments; and, + Must be knowledgeable of best practice clinical treatment necessary to deliver effective therapy interventions. + Must be knowledgeable of best practices for substance use disorder and an approach that promotes many paths to recovery. Abilities: + Ability to apply knowledge and communication to age specific and/or population specific care competencies + Ability to collect information from consumer and family members to determine nature and extent of consumer needs. + Ability to develop individual plan of treatment for each consumer + Ability to conduct counseling appropriate to consumer needs. + Ability to document the therapeutic process EmpowerHer Job Description Addendum The EmpowerHer Program requires professional who are passionate about supporting women and their families with a specific interest in maximizing support for those who have been historically underserved.
