Aspirus Wausau Hospital in Wausau, WI is seeking a Interventional Radiology Clinic Navigator/Patient Care Coordinator to join our team!

The IR Navigator/Patient Care Coordinator is a clinician who functions as an active health care advisor and advocate throughout the process of screening, diagnosis, and treatment of Interventional Radiology Care. Primary responsibilities include developing and providing patient education, support and answering questions about procedures and studies performed as part of the care processes in the IR Clinic/ Procedures. Additionally, responsibilities involve aiding with prior authorizations and collaborating with the Radiology Prep Team to administer IVs and oversee patients in various modalities as required. This includes informing patients of the IR clinic results and arranging appropriate follow up. They schedule patients for procedures, clinic visits, and imaging exams. This person will also coordinate interventions as needed to meet patient’s emotional and or social needs. They will reach out to other departments and/or agencies to help facilitate patient and family physical, emotional and /or social needs. Some examples of this include home care agencies, social services, and transportation services.

The IR Navigator/ Patient Care Coordinator is a professional who assumes the responsibility and accountability for the provision of care including assessment, planning, implementation and evaluation of the Interventional Radiology patient that incorporates evidence based practice. IR patients present with a variety of system diagnosis, including but not limited to Vascular, Gastrointestinal, Genitourinary, Respiratory etc. The Navigator/Patient Care Coordinator must have a knowledge base for all systems. This can be a RN or Registered Radiologic Technologist. The RN will have oversight of the technologist for RN duties that can be delegated.

HOURS: Full-time or .9 FTE; 72 hours bi-weekly; varied shifts

Experience/Qualifications:

Identifies opportunities to improve patient experience. Participates in projects/programs designed to enhance service delivery and patient satisfaction Identifies opportunities to facilitate cost-effective experience and improve Environment of Care and patient and staff safety. Provides expert clinical care at point of service Participates in projects/programs designed to enhance service delivery and patient satisfaction and create a save environment to avoid error Collaborates with health care providers to offer seamless care delivery Actively participates as a member of a multidisciplinary team that coordinates Interventional Radiology clinic patients care, interventions and follow up care and to identify issues and solutions to optimize patient outcome. Coordinates care of patients in a holistic, empathetic manner, providing personal and family consultations and educational resources, as indicated. Establishes short and long term goals reflective of the overall plan of care. Defines and alter protocols/ procedures to meet care needs of individual patient. Oversees and organizes coordination of clinical findings, including providing education to the patient and family about various interventional procedures. Arranges for additional support and follow-up as appropriate. Under direction of Radiologist, communicates and discusses study and biopsy results with patients. Informs them of additional procedures or studies ordered by Radiologist or which normally would occur as part of diagnosis and treatment process. Works with imaging personnel to oversee patient vitals in alternative modalities as required by imaging leadership. Tasks may encompass initiating IVs, monitoring patient vitals during MRI/CT scans, and similar duties. Reviews referred procedures with radiologist as appropriate prior to scheduling the patient Schedules procedures, exams and visits for the IR clinic, IR procedures and Imaging exams. Review clinicals/diagnostics to ensure referred procedure correlates with clinicals/diagnostics. If there are discrepancies, contacts referring physician for clarification Ensures imaging and imaging reports are available for radiologist review. Contact outside facilities to obtain if they were completed at an outside facility. Reviews the patient’s medications. Follow SIR guidelines for any anticoagulants the patient may be on. Coordinates with physician ordering the anticoagulant to ensure the medication is held appropriately. Obtain preauthorization for procedures ordered by Interventional Radiologist or as directed by imaging leadership to maintain seamless continuity of care Resource for CPT codes for procedures referred by other Physicians to be completed by IR.

Education:

Knowledge of patient care standards and practices normally acquired through completion of a nursing or Radiologic Technology program. Bachelor’s Degree in Nursing or Registered Radiologic Technologist

Experience:

Previous Interventional Radiology Experience Preferred

Required:

Possess an active Wisconsin RN Nursing License or active Wisconsin Radiologic Technologist license Current BLS certification or obtained within 90 days of hire - All clinical personnel in identified job codes are required to maintain Basic Life Support (BLS) training through independent study, online manikin and knowledge testing. An acceptable training program is the Basic Life Support (BLS) Health Care Provider course from the American Heart Association or the American Red Cross Professional Rescuer Program with AED. (See Cardiopulmonary Resuscitation Training Policy). Current ACLS certification or obtained within 90 days of hire.
