Job Opportunity: Remote Nurse Practitioner/Family Medicine!
Location: Remote, Cedar Rapids, IA (Candidates should live close to the targeted city and may occasionally be asked to visit patients’ homes for specific situations, though this is rare.)

Board Certified State licensure in IAPrevious Experience Desired:
2 years of direct patient care experience related to polychronic conditionsStart Date: ASAP
Duration/Schedule: 3 months with likely extensions
On-call/Callback: No call required
Types of Cases/Procedures:
Maintain quality of care standards Assist patients with enrolling in educational programs Knowledge of disease diagnosis and prevention Assess patients’ health status Implement and follow up on care plans Document patient information accurately and timelyPatient Volume: Complete 3+ patient visits daily
Additional Notes:
This role primarily involves virtual work with occasional travel within a specific geography for in-home assessments. Training includes 3 days onsite and the remainder virtually.
