Job Description:
The Licensed Clinical Social Worker (LCSW) or Certified Social Worker (CSW) provides certified
social work services to patients and their families. This LCSW or CSW is a dedicated position
responsible for coordinating discharge planning and case management of patients within the hospital setting. This involves hands-on interaction with aiding patients, in conjunction with family members, in achieving an effective transition from hospital to post-hospitalization care through assessment, planning, relaying information, and referral to appropriate community agencies throughout the
discharge planning process. The LCSW or CSW provides advice or guidance for any other nonmedical problem or barriers to care that may hinder the patient’s rate of recovery or delay discharge
and serves as a resource for continuity of care options after discharge. The LCSW or CSW serves as a qualified medical healthcare provider (QMHP) for patients in mental inquest warrant (MIW) hearings. The LCSW or CSW also provides clinical services to patients regarding their mental health needs. This position requires a state license to practice clinical social work.
Requirements:
Master’s Degree in Social Work from a school of social work accredited by the Council on Social Work Education required.
Certification and/or license in Social Work in the state of Kentucky.
Three years of experience social work experience required.
Preferred Qualifications:
Experience in medical social work preferred.
A background in behavioral health preferred.
