The Lead Care Coordinator is a member of the multidisciplinary team that actively facilitates those functions that move a patient through an acute episode of care and links to internal and external resources and the payor community. The Lead Care Coordinator leads the process which identifies complex outlier patients and plans, implements, monitors, and measures the effectiveness of more complex interventions to meet patients' treatment and transitional needs. Requirements of the job may include support to Lead Utilization Review nurse and Utilization Review team. The Lead Care Coordinator acts as an advocate for the shift, making sure the Clinical Manager/Director are aware of the unique needs of the shift. The Lead Care Coordinator providers leadership for personnel by being a team leader, serving as a preceptor and being an active member of the health care team,

Responsibilities

Care Coordination/Transition Planning
* • Proactively screen and assess the acuity and transitional needs of the patient and support the Care Management team in their work with complex and outlier patients, reviewing and working with the Care Coordinator to address barriers and obtain needed resources to progress toward discharge.

* Partners with the Interdisciplinary team to facilitate placement in a rehab, SNF, sub-acute, assisted living facility, or home with Home Health Care, as well as arrange DME and other services as ordered.

* Identify appropriate consultative services that would enhance patients timely transitional plan.

* Encourages and facilitates patient and family participation in all care team decisions.

* Collaborates with post-acute services, care managers, and PCPs to ensure transition back to the home environment.

 
People Management
* Assist in adjustment of daily assignments based on staffing and acuity of caseload.

* Demonstrated knowledge of NAH and department policies and procedures. Works with other departmental leadership to ensure compliance of best practice among each staff member.

* Establishes and maintains mutual trust and respect with staff.

* Promotes an environment that motivates, nurtures, and empowers staff. Supports department communication lines through the use of huddles, department meetings and other means of fostering employee engagement.

* Works with employees to facilitate and achieve skill, educational and performance goals.

* Addresses workflow issues, ensures incidents are appropriately documented and corrective steps are taken.

* Embraces an improvement mindset and works with departmental leadership on identification and implementation of process improvement initiatives.

* Advocates for both patients and staff as needs arise.


Resource Management
* Evaluates the appropriateness of care delivery in the inpatient setting and communicates any discrepancies with the medical team.

* Facilitates the management of care in an outpatient setting.

* Takes a leadership role in identifying opportunities to reduce risk both financial and clinical, through analysis and resources consumption outcomes.

* Assertively manage resource utilization while appropriately navigating patient movement along the continuum of care.

 
Compliance/Safety
* Stays current and complies with state and federal regulations/statutes and company policies that impact the employees area of responsibility.

* If required for position, ensures all certifications and/or licenses are up-to-date and valid prior to expiration dates.

* Completes all company mandatory modules and required job specific training in the specified time frame.

* Responsible for reporting any safety related incident in a timely fashion through the Midas/RDE tool; attends all safety related training programs; performs work in a safe manner; monitors work environment for possible safety issues and ensures others are also performing work in a safe manner.

Qualifications

Education
Bachelor’s Degree or Master's of Social Work - Required

 

Master's Degree in Nursing- Preferred

 


Certification & Licensures
RN license in the State of Arizona- Required; or for applicants with an MSW, LMSW or LCSW- Preferred

 

Case Management Certification (ACMA, CMSA, CCMC)- Preferred

 


Experience
1+ Years experience in acute care management - Preferred

 

Healthcare is a rapidly changing environment and technology is integrated into almost all aspects of patient care. Computers and other electronic devices are utilized across the organization and throughout each department. Colleagues must have an understanding of computers, and competence in using computers and basic software programs.
