A Los Angeles based Community Health Program with a mission of increasing the well-being of individuals impacted by substance use disorder is in search of a passionate Case Manager to join our team!

REQUIREMENTS:

High school diploma or equivalent required.1+ year of experience with intensive case management services (may be substituted with an associate's degree in human services or higher)Experience with vulnerable populations

RESPONSIBILITIES:

Meet with participants regularly in order to assess needs and link to services and care including need for medical care, shelter, food, clothing, and harm reduction services/supplies.Develop and implement individualized initial assessments and care plansEducate participants on the range of services available to themAssist participants in entering service programs.
